Hi,

As many of you will be aware that Indirapuram Public School has been made polling booth for Crossings Republik & Since 18th Sep 16 camp is being organised on Every Sunday at Indirapuram Public School, which will continue till October end. 

Request all the resident to check your name in voter list ( http://ceouttarpradesh.nic.in/ ) and whoever are not enrolled are requested to apply at these camps. This exercise is for enrolment of new voter hence if your name is enrolled and you have not received Voter ID than you need not submit it again.

Current Voter list also attached. Name of CR Voters are appearing in 21,21,22 few name may be there in 18 & 19 too. Better to check online.

Have observed that there are common error in many form due to that correction was required.
Important point while filling form for enrolment in voter list.
1) Form 6 as shown in attached pic is required to be filled in, old forms are not accepted.
2) 2 Pic required one to be affixed at given place, One extra to be stapled.
3) Age to be mentioned as on 1st Jan 2017
4) Name and address to be mentioned in Hindi and English Both. Also write mobile no. and email id.
5) On 2nd page under Sr. No. IV, date from which living at current address to be written.
6) Mentioning Previous address is compulsory irrespective of you are enrolled at that address or not.
7) If Voter id was made at last address same need to be mentioned else skip
8) On 3rd page Name and Address to be mentioned at 2 place.
9) Documentary Proof for Age, Address & ID is required.
10) For one family one person can visit and submit filled in and signed form. BLO were not accepting forms other than family member.
