Hi!

The hospital must have given you a receipt. You have to visit Collectrait office (Jan Suvidha Kendra) and there will be a form available at the counter (Carry all your I'd and address proofs - originals photocopy). The same form is available online on the website. You can fill the same first and then shall visit the office.

As far as I can recall, the form needs to be submitted within 30 days ( I am not sure). Else you would need CMO's office stamp and signature along with affidavit.

They would also see the originals at the counter, at the time of submitting the form and would give you an electronic receipt. The status can be traced online and the certificate can be collected within 15 days.
